Declaration:

1. I hereby declare that all the information provided above is true and correct to the best of my knowledge and belief.

2. I declare that, prior to this date, I have never been involved in any criminal, financial, or social offense, nor have I ever been convicted or punished for any such matter. I further declare that I maintain the image of a hardworking, responsible, honest, and dedicated social worker in society, and I am deeply committed to social service. Serving society under the guidance and protection of Just Help Me Foundation is my ultimate goal. If any part of this declaration is found to be false, my membership shall be deemed automatically terminated.

3. The fee deposited by me shall not be refunded under any circumstances, nor shall it be adjusted in any manner. In the event that any action is taken against me due to my improper or unlawful conduct, I shall not claim a refund of the membership fee.

4. If, under the instructions of the organization, I fail to complete the formation of members under my supervision within the prescribed time limit, my nomination letter or appointment letter shall automatically stand cancelled. I willingly accept this condition.

5. I pledge that I will always work honestly, responsibly, and in a disciplined manner toward the objectives and activities of the organization. I shall strictly follow the guidance and directions of the National President and senior office bearers. I will remain committed to supporting and protecting truth, the weak, and those who are oppressed by society or administrative authorities, and I will continuously strive for their welfare. Any warning or disciplinary action taken by the National President against me for acts contrary to the objectives and rules of the organization shall be accepted by me as a disciplined member.
